To fill out this DOT Application, you will need to know the following:

           Social Security Number

           Home address history for the past 3 years

           Current driver license number and driver license history for the past 3 years

           Employment history for previous 10 years – No gaps in employment history - If you were unemployed or worked in a different field/industry that was non driving, list this on the application.

           History of traffic accidents, violations and/or convictions from the last 5 years (including DUI or reckless driving conviction and license suspension)

           Copy of your driver’s license – front and back can be uploaded at the time of applying

           Copy of your current medical card – can be uploaded at the time of applying

Required entry fields are followed by *, meaning you must provide the requested information to continue. In addition, you will provide an electronic signature when the application is complete, if you do not E-sign the application the information will not be transmitted to us. If you are unable to finish the application, you can return later by simply hitting the return to application link and you will be directed to the section that you left incomplete.
